Do Lip Fillers Hurt? What to Expect During Treatment

What lip filler actually feels like

Lip filler is injected with a very fine needle (or a blunt-tip cannula) into the soft tissue of the lips. Without numbing, you'd feel a quick pinch followed by a pressure or stinging sensation as the hyaluronic-acid filler is placed. The lips are sensitive, but the injections are shallow and each one takes only a few seconds.

Patients usually rate the sensation a 3–4 out of 10 once numbing is applied — comparable to a brief pinch and pressure rather than a sharp pain.

How we keep you comfortable

  • Topical numbing cream applied to the lips 15–20 minutes before treatment to dull the surface nerves.
  • Lidocaine in the filler itself — most modern lip fillers (Juvéderm, Restylane Kysse, RHA) contain lidocaine, so the area numbs further as the first drops are placed.
  • Dental blocks on request for patients with high sensitivity, fully numbing the upper or lower lip the way a dentist would.
  • Ice and vibration at the injection site to distract the nerves and reduce sting.
  • Slow technique — small aliquots, brief pauses, and steady pressure between injections.

What recovery feels like

Most patients leave the appointment with some tenderness, mild swelling, and the possibility of small bruises at injection points. The lips often feel firm or "full" for the first 24–48 hours, then soften as the filler integrates over the next 1–2 weeks.

To minimize discomfort and swelling afterward:

  • Apply a cool (not frozen) compress for 10 minutes at a time during the first day.
  • Sleep with your head slightly elevated the first night.
  • Avoid strenuous exercise, alcohol, and high heat (saunas, hot yoga) for 24 hours.
  • Skip blood-thinning supplements like fish oil and high-dose vitamin E for 24 hours.
  • Stay hydrated — hyaluronic-acid filler binds water, so it works better when you are.

Who tends to feel it more

Pain tolerance varies, but a few factors make the experience more sensitive: getting injections during your menstrual cycle, going in dehydrated or under-slept, and previous cold sores in the area. If you've had cold sores, ask about a short course of antiviral medication before your appointment.
